Support Coordinator

Job Description:

The Support Coordinator position is designed to support individuals in the foster care system. The primary goal of this role is to ensure that the young people in foster care receive the necessary support, guidance, and resources to help them transition into adulthood successfully.

 

Responsibilities:

  1. Develop and maintain strong relationships with the youth, their families, and other stakeholders involved in their care.

  2. Assist with the coordination and implementation of individualized service plans for each youth in the foster care system.

  3. Monitor the progress of youth in the program and provide regular updates to relevant parties.

  4. Conduct case management assessments and risk assessments to determine the appropriate level of support needed for each individual.

  5. Connect youth to community resources and services that will aid in their personal growth and development.

  6. Collaborate with other professionals, such as therapists, teachers, and social workers, to ensure a holistic approach to supporting the youth.

  7. Attend and participate in team meetings, case conferences, and other relevant events to advocate for the best interests of the youth.

  8. Maintain accurate and up-to-date records of all case management activities and ensure compliance with relevant regulations and guidelines.

 

Qualifications:

  1. Bachelor’s degree in social work, psychology, or a related field

- OR-

          Minimum of two years of experience in case management, social services, or a related field.

  1. Must be at least 21 years old

  2. Have the ability to pass a background check

  3. Strong communication and interpersonal skills.

  4. Ability to work effectively in a team and independently.

  5. Strong organizational and time-management skills.

  6. Commitment to promoting the well-being and success of individuals in the foster care system
